2016 Ohio State Taxes
To file your Ohio and federal taxes, please bring these documents:
All documents on the 2016 Federal Taxes Checklist
Ohio tax forms:
Any estimated income tax payments submitted on the following forms
IT 1040ES
IT 40P
SD 100ES
SD 40P
2015 IT 1040
You need this to determine any individual income tax overpayment that should be credited to 2016.
2015 SD 100
You need this to determine any school district tax overpayment that should be credited to 2016.
All other income, expense, and tax documents from 2016 for you and/or your dependents, including:
IT 2
Employee wages earned and taxes withheld
Documents for business income and expenses relating to Ohio
Documents for rental income you received from any property in Ohio
Disability or survivorship benefits documents
Documents for unreimbursed premiums for long-term care insurance plans and unsubsidized health insurance plans
Documents for job loss and job training courses taken to improve chances of getting a new job
Documents for contributions made to:
College Advantage 529 savings plans
Political campaigns
Individual Development Accounts (IDAs)
STABLE accounts (Ohio's ABLE plan)
Members or beneficiaries of members of the Ohio National Guard should bring documents showing:
Reimbursements of life insurance premiums
Payments of death benefits
Documents for lump sum distributions from retirement or employer plans
Documents for goods and services bought from a retailer outside of Ohio
Ohio Historic Preservation Tax Credit Certificate
Documentation of Organ Donor Expenses
Documentation of Pell Grant or Ohio College Opportunity Grant funds
If funds were used to pay expenses other than qualifying tuition and fees
Other information and documents
Copies of prior year federal Forms 1040 and Ohio IT 1040 (if you were paid back for any items you took a deduction for on those returns)
Include any schedules or forms attached to your federal Form 1040
Prior year LS worksheet
The worksheet used to calculate the Lump Sum Distribution and Lump Sum Retirement Income Credits
Information on school districts that you lived in this year
You need the dates of when you lived in each school district, if you lived in more than one during the year
Information about years of military and federal civil service (if applicable)
Documentation of receipt of military injury relief funds (if applicable)
